How do you get out of a vehicle that is completely submergered underwater?

So I've had this burning desire to figure this out. If you find yourself in a vehicle completely submerged underwater, what is the correct way to get out without injuring yourself? And is there a difference from doing it in a car or a truck? How would the pressure of the water effect this?
